Hospice

Hospice helps terminally ill patients continue to live with independence and as free of pain as possible. Families are provided physical, emotional and spiritual support to better care for their loved ones, most often at home.

Trained staff and volunteers serve as an interdisciplinary team with your personal physician, evaluating specific needs and establishing an individualized plan of care.

  • Medical Director coordinates care as ordered by your personal physician.
  • Registered Nurses coordinate all aspects of care, with management of pain and other symptoms to help patients remain alert and comfortable.
  • Certified Nursing Assistants help with personal care such as bathing, exercise, grooming and diet.
  • Medical Social Worker provides supportive counseling, bereavement care and follow up. Identifies resources for legal and financial aid.
  • Physical and Occupational Therapists provide home safety evaluations, education and pain management.
  • Trained Hospice Volunteers offer comfort and support to patients and family members.
  • Hospice Chaplains help meet spiritual needs.

Home Health Care

Home Health Care provides medical services in your home, under the direction of your personal physician. This compassionate care promotes independence and enhances healing and comfort. Home Health Services include:

  • Intermittent Skilled Nursing Aids
  • Medical Social Services
  • Physical, Speech & Language Therapy
  • Occupational Therapy
  • Intermittent Certified Nursing
  • Nutritional Counseling
  • Bereavement Counseling

Personal Care Services (PCS)

This program assists adults and children with physical and developmental disabilities as well as the elderly. Trained, caring staff provide personal care, transportation and assistance with grooming, exercise and household duties. PCS helps patients meet their physical challenges and continue living in their own homes.
PCS Eligibility:

  • Patient is eligible for Medicaid
  • Patient has a physician's statement
    that Personal Care Services are required
  • Patient is able to stay at home safely
